Value-based care is reshaping how physicians are paid and how patient outcomes are measured — shifting the focus from volume of services to quality of care. KCMPA has entered into value-based care arrangements on behalf of our independent member practices, giving you access to the scale, data and payer relationships that are typically out of reach for practices operating on their own. These partnerships are designed to help you meet quality benchmarks, strengthen your standing with payers, and share in the financial upside of better patient outcomes — all while preserving your independence. Below are summaries of KCMPA's current VBC arrangements.

ACO Value-Based Care Partnership (Vytalize Health)

KCMPA has partnered with Vytalize Health to bring our independent member practices into a Medicare Shared Savings Program (MSSP) Accountable Care Organization, with participation beginning in Performance Year 2027. Through this partnership, member practices gain access to Vytalize's clinical care management infrastructure, data analytics and care coordination resources — the same tools larger health systems use — while remaining independent. Vytalize's care teams will work alongside member practices to help identify patients who could benefit from closer follow-up, coordinate care after hospital stays, and support patients in navigating tests, referrals and chronic condition management. KCMPA negotiated the terms of this partnership on behalf of members, with specific protections around practice autonomy, data transparency and fair terms for participation. As we move through implementation, we'll share more detail on how the program will work for your practice and your patients.

Participating through KCMPA gives your practice the benefit of our collective negotiating position — typically including a stronger share of shared savings than practices typically secure going it alone.

Humana Medicare Advantage Value-Based Agreement

KCMPA participates in a Medicare Advantage value-based (shared savings) agreement with Humana, extending value-based care principles to our members’ Humana Medicare Advantage patients. Through this partnership, KCMPA meets monthly with Humana’s care teams to review practice performance on quality and utilization metrics — many of the same measures tracked through our ACO partnership — helping keep member practices informed and engaged in how their patients are performing under the arrangement.
